Please provide an active and valid email address, and an active phone number. We recommend using an email that you have easy and quick access to, especially on your mobile device. We will need a way to send documentation back and fourth if need be, and being able to quickly access documentation or information is vital to you resolving your claim.
We will need to speak with you on the telephone throughout the entirety of the claim process, we recommend using the number of a phone you currently are using everyday. If this email and number was used at the time of purchase, they will be required to match or your claim will be denied.
Please provide details about the transaction below, we will use this information to verify the validity of the information provided about the sale/transaction with-in this claim by cross referencing with information about the transaction with the payment processors.
